ND Transistion and Diversion Services Pilot Project (ND TDPP)
Formerly known as ADRL, ND TDPP assists individuals with a disability, transitioning an individual from a provider operated residential setting into community living.

Services Offered:

  • Transition Assistance: Helping individuals with disabilities move from group settings to community homes.
  • Diversion Support: Preventing long-term institutionalization.
  • Financial Help: Up to $1,500 for needs like deposits and furniture.
  • Housing Assistance: Support in securing or maintaining housing.
  • Short-Term Services: Available for 90 days after transition or diversion

Eligibility:

  • On or eligible for Medicaid/Medicaid Expansion.
  • Significant disability, including mental illness or substance use disorder.

Current Living Situation:

  • To qualify for the program, participants generally need to be in an institutional setting that the program aims to transition from or divert.
  • This includes environments such as:
    1. Nursing Homes
    2. Assisted Living Facilities
    3. Similar Settings
  • Examples of such institutions are:
    1. Centre Inc.
    2. Bismarck Transition Center Heartview
    3. Sharehouse
    4. Prairie St. John’s
